Patient Visit Toolkit

Prepare before the doctor visit, so the consultation becomes clearer and safer

Many patients forget important details during a short consultation. This toolkit helps them organize symptoms, medicines, reports, red flags, and questions before they meet a qualified clinician.

Start checklist
1

Write the main problem

Use simple words: pain, fever, weakness, swelling, breathing difficulty, vision change, bleeding, numbness, or another concern.

2

Record timing and pattern

Note when it started, whether it is improving or worsening, what triggers it, and what gives relief.

3

List medicines and allergies

Bring current medicines, supplements, previous prescriptions, and any allergy history if available.

4

Bring reports and images

Keep lab reports, imaging films, discharge papers, previous diagnosis notes, and operation records together.

Do not wait when warning signs are present

Seek urgent qualified medical care for severe breathing difficulty, chest pain, signs of stroke, severe injury, uncontrolled bleeding, confusion, fainting, severe allergic reaction, sudden severe pain, or rapidly worsening symptoms.

Ask better questions

Questions to ask the doctor

Patients can print or copy these questions and add their own. Clear questions often lead to clearer diagnosis, safer treatment, and better follow-up.
